Declaration for the purpose of exempting training from VAT*

I declare that the above-described training service is in the nature of a vocational training or retraining service, as referred to in the Law of March 11, 2004 on Value Added Tax (Journal of Laws 2023.0.1570, as amended) and the service recipient (mark appropriate):

is a taxpayer of goods and services tax – therefore participation in the training is taxable under Art. 28b point 1 of the Law of March 11, 2004 on Value Added Tax (Journal of Laws 2023.0.1570, as amended), the place of supply of services is the place where the taxpayer who is the recipient of the services has its headquarters of business activity, with the provision of paragraphs 2-4 and Article 28g paragraph 1, the place of supply of services related to cultural, artistic, sports, scientific and other activities.is a private person who is not a taxpayer on goods and services - therefore participation in the training is taxable under Article 28c of the Act of 11.03.2004 on tax on goods and services, with the provision of Article 28g paragraph 1, the place of supply of services related to cultural, artistic, sports, scientific and other activities.

What are the conditions for participation in the training?

The condition for participation in the training is to complete and send the application form, the confirmation of receipt of which by the Organizer constitutes the conclusion of the training participation agreement, subject to points 2-18 below. By applying for participation in the training, the applicant undertakes to pay the agreed fee for the training.

Can I cancel my participation in the training?

Cancellation of an application made remotely is possible within 14 calendar days from the date of application and not less than 14 days before the date of commencement of the training, provided that the Organizer is informed in writing (including by e-mail). Cancellation after the indicated date will result in a charge for the full fee.

I want to take part in the training, but something happened and I can't take part on this date. What can I do?

The applicant has the possibility to move the date of the training to another date once (if there are free places available), or to use the paid amount for another training of the Organizer, provided that the difference in price is compensated and this move is made no later than 14 days before the planned date of the training. The payment is not refundable, neither in part nor in whole. Lack of participation in the training cannot be understood as a withdrawal from the agreement and does not release from the obligation to pay the price of training.

If I cannot attend the training, can someone from my workplace replace me?

It is possible to replace the Participant with another person than indicated in the Form. In order to make such a change, the Applicant is obliged to notify the Organizer by e-mail about the need for such a change (to the address: csm@csmedu.pl) no later than 3 days before the beginning of the first day of the Training. Due to the substantive content of the Training and high standards of training quality, such a change can be made only with respect to the entire time dimension of the Training (and not its selected part). To receive a certificate of completion of the training, the participant should participate in the entire training - in its full hour.

What VAT rate applies to payments for participation in the training?

The Organizer informs that the training fees will apply the 23% VAT rate, unless the Applicant includes a declaration in the Application that he or she is a VAT payer, then, in accordance with applicable regulations, the fee is subject to the reverse VAT rate.

Is it possible to reduce the price for training?

Participation in previous editions of training by a given Participant entitles him to receive a one-time discount of 4% for the next training. If the Participant wishes to use the discount, it is necessary to indicate the training in which the Participant participated previously when registering for the next training. Discounts do not add up. Discounts can only be used by the Participant and cannot be transferred to third parties. Discounts are not exchangeable for cash or a refundable transfer and its unused portion is not refunded. Discount rights received in a calendar year can be used until the end of the next calendar year.
